  • Anterior capsule fibrosis is thought to be the resultant of lens epithelium proliferation.
  • It has also been reported more commonly with silicone foldable lens implants.
  • May complicate:
    • Continuous curvilinear capsulorhexis in an uncomplicated phacoemulsification
    • Eye with disrupted zonules following trauma
    • Small diameter capsulorhexis or eye with pseudoexfoliation
  • Complications: reduce vision, zonular stretching, IOL compression, IOL dislocation, ciliary body traction or hypotonous choroidal effusions.
  • Preventive measures:
    • Complete anterior capsule vacuuming at the end of cortical aspiration
    • Bigger diameter capsulorhexis
    • Use of single piece PMMA lens with a relatively broad-expanded haptics structure
  • Treatment: Neodynium: YAG laser anterior capsulotomy for significant anterior capsule contracture.
